Before we can arrange your consultation, we kindly ask you to complete our Patient Consent Form.

Please note:

By requesting medical services from our clinic, you acknowledge that you have read, understood, and agreed to these terms and conditions.
Your consultation will be arranged only after we have confirmed both your completed consent form and your deposit payment.
The deposit will be applied toward your final medical bill.
If the total treatment cost exceeds the deposit amount, the remaining balance will be charged after your consultation. If the final amount is less than the deposit, the difference will be refunded.

안내 사항

당원에 진료를 요청하시는 경우, 본 동의서 및 관련 이용약관의 내용을 읽고 이해하였으며 이에 동의한 것으로 간주됩니다.
작성된 환자 동의서와 보증금 결제가 모두 확인된 후에 진료 예약 및 의료진 배정이 진행됩니다.
결제하신 보증금은 최종 진료비에서 차감됩니다.
실제 진료비가 보증금을 초과하는 경우에는 진료 후 차액을 추가로 결제해 주셔야 하며, 실제 진료비가 보증금보다 적은 경우에는 차액을 환불해 드립니다.
